If you think you meet the requirements, call Family Services at (859) 252-2224. If you do meet all requirements based on a telephone screening then you will schedule an appointment to complete a pre-application. Please note the following:
Allow 30 minutes to complete the pre-application
A registration fee of $15 is required
The pre-application will include: Name(s), birthdays and social security number(s) of household members, income sources for entire household, and bankruptcy information/discharges
You must give approval for a credit report
If your pre-application is approved, you will need to schedule an additional appointment to complete a full application. Please note the following:
Allow a minimum of one hour to complete the full application
All applicants must be present (i.e. spouses)
A form will be sent out to landlords, employers, etc. to verify the accuracy of the information provided
Bring pay stubs from the previous two months and taxes for the last 2 years
Bring documentation of any other sources of income (i.e. child support, social security, etc.)
